Active Connect replaces fragmented, appointment-driven follow-up with a governed rehabilitation pathway. Patients move through a defined programme with structured support, clear milestones, and access to your physiotherapy team as clinical needs arise.
Step 1: Pre-Operative Preparation
Before surgery, you will be provided with tailored information about what to expect in hospital and as you recover. Starting rehabilitation before having your surgery will mean you will arrive in hospital informed, prepared and ready to recover.
Step 2: Structured Home Rehabilitation
After discharge you will progress through a defined rehabilitation pathway which includes a detailed, exercise program with milestone-based progressions and a clear understanding of what to focus on at each stage of recovery.
Step 3: Straightforward Access & Escalation
Contact your physiotherapy team via secure messaging or online video consultation when you need assistance. Your concerns, whether relating to your pain, movement, wound or general progress, are reviewed promptly and escalated to the right clinical team when required.
Step 4: Proactive Education & Monitoring
Education resources are delivered to you, depending on your stage of recovery. Our resources are tailored by expert clinicians, and are based on decades of experience of working with people following surgery.
Step 5: Measuring outcomes — PROMs & PREMs
Patient-reported outcome and experience measures are embedded throughout our pathway. This helps you to understand how you are progressing, helps your physiotherapy team make clinical decisions about your care and drive continuous improvement in our service delivery.
